Submit a Request for Accommodations

The first step in seeking accommodations is disclosing your disability. The process for doing this is outlined below and includes submitting documentation and meeting with the ADA coordinator to engage in a collaborative discussion about accommodations and the accessibility resources available at Covenant College.

Incoming Students: A representative from our office will reach out to students who complete the Intake Form to schedule a time to connect during one of the student's on-campus visits or to meet remotely ahead of the start of the semester. To receive accommodations for a documented difference or disability, the student must complete the following 3-step interactive process:

This Request Form includes basic information about the student and a brief description of the student’s difference or disability and requested accommodations.

Documentation must:
Clearly state the nature of the diagnosed disability or learning difference
Be recent enough to speak to the student’s current needs (within the last 3 years unless the impact of the disability is stable over time)
Be written and signed by a qualified professional

Provide a list of recommended accommodations

Recommendation letters from care providers will be reviewed, but may not be sufficient to approve accommodation; therefore, we ask your care provider to complete the forms below, or include the information that is asked in them. Academic Accommodations:
If requesting academic accommodations, a psychoeducational evaluation or neuropsychological evaluation is preferred and sometimes necessary in order to establish your disability/difference. If your evaluation is outdated, we may need supplemental documentation. An IEP or 504 plan does not alone constitute documentation of a disability but may be included as relevant background information. An Academic Verification Form can be used as helpful documentation in establishing a disability.

Dining Accommodations:
If requesting dining accommodation, your care provider is welcome and encouraged to complete the Verification for Alteration to Meal Plan to accompany your request. Allergy test results are not considered adequate documentation.
 
Housing Accommodations:
If requesting a housing accommodation, your care provider must complete the Verification Form for Housing to accompany your request.

The student’s self-report is a vital source of information in best understanding how our office  can support him or her. For that reason, accommodations will not be activated until the student-client has met with a member of our staff.

Submitting a Complaint:  
A complaint of discrimination on the basis of disability should be filed in writing to the ADA Coordinator at disabilityservices@covenant.edu within 30 calendar days of the allegedly discriminatory act. You may include a clear description of the complaint: what happened, who was involved, when and where it happened.

Denials and Appeals: 
A student who feels as though they have been denied a reasonable accommodation or is dissatisfied with the determination that they are not eligible as an Otherwise Qualified Individual may appeal in writing.

Appeals must be made in writing within five (5) days of notification of the denial to the Section 504 Appeals Coordinator, Nesha Evans (nesha.evans@covenant.edu). It would be helpful to include:

  • Date/time/location of occurrence
  • What approved accommodation you believe was not adequately provided
  • Any other relevant details or context
  • Desired next steps, if applicable

The Section 504 Appeals Coordinator will schedule meeting(s) with the various individuals involved in the process. The Section 504 Appeals Coordinator will render a decision in writing. The decision of the Section 504 Appeals Coordinator is final.

The availability and use of this Covenant College Appeal Process nor the Complaint Process does not prevent a student from filing a complaint of discrimination with external agencies such as the U.S. Department of Education, Office for Civil Rights (OCR). That office will determine whether a further investigation is warranted.  You may find information on how to file a complaint with OCR here.
